Dr. Rateb, is the intention (Niyyah) something man feels or should he utter it using specific words?

No need to utter the Niyyah.
When the time of Salat (prayer) arrives, uttering the Niyyah for Wudu' is just an addition. Some scholars permitted it, but it is an overdoing.
When you set the alarm clock in order to wake up in Suhoor to eat for the purpose of fasting the next day, in my opinion uttering that you intend to fast next day out of sincerity of Faith and in the hope of earning reward is not necessary, because the heart is the place of Niyyah , and it does not need to be uttered (setting the alarm clock is the Niyyah in the heart).
Again, some scholars permitted uttering the Niyyah by saying, "I intend to fast tomorrow".  
In my opinion uttering Niyyah for Wudu', Salah or Siyaam is not necessary, because the heart is the place of Niyyah. There is no harm in uttering Niyyah, but it is not a necessity.
